To request funds from the Connecticut Office of Rural Health (CT-ORH), an application must be completed and submitted to the CT-ORH. Applicants must meet the eligibility criteria and comply with the application instructions and guidelines.
Funding Information
Sub-award funds come to the Connecticut Office of Rural Health (CT-ORH) through the Department of Health and Human Services via the Federal Office of Rural Health Policy. Awards are contingent upon federal funding to the CT-ORH. Funding is available to assist CT rural community agencies/ organizations, rural hospitals, rural clinics, and rural health care provider groups or organizations with specific initiatives that support and positively impact access, quality and costs with a goal to increase program or organizational efficiency and utilization of resources to avoid duplication of services.
Grant initiative activities should be evidenced-based and/or aligned with Affordable Care Act initiatives. The organization must have a program currently operating or impacting any of the following focus areas:
- Emergency Medical Services/preparedness
- Mental/Behavioral Health
- Substance Use Disorder
- Telehealth
- Transportation
- Workforce Development Programs
Funds may be used for:
- Program development, enhancement, implementation or evaluation
- Health program education or training
- Strategic planning for program sustainability
A collaborative or coordinated community, regional or state rural health care partnership is required for the proposed grant initiative. The applicant must describe the partnerships or collaborations and the role of each partner organization toward the success of the proposed project. Funds cannot be used for direct patient health care services, purchasing equipment, lobbying activities and construction or remodeling of facilities.
Funds will be issued on a reimbursement basis only, must comply with contract deliverables and may only be used for the objectives for which the grant was awarded.
Application Process and Submission
All funding requests will be reviewed by the CT-ORH with additional consultation as needed. Application funding requests must be typed and submitted electronically on the organization's letterhead as one document to lfedewa@nwcc.edu.
