2009-10 CT-ORH Rural Healthcare Initiatives Grant

Program Announcement

The Connecticut Office of Rural Health (CT-ORH), through the administration of its Rural Health Office Grant program, announces the availability of grant funds for rural Connecticut healthcare initiatives. The grant funds come to the CT-ORH through the Department of Health and Human Services through the federal Office of Rural Health Policy. Grant awards are contingent upon federal award funding to the CT-ORH.

Purpose and Eligibility

The purpose of these competitive grants is to assist CT rural community agencies/organizations, rural hospitals, rural clinics, and rural health care provider groups or organizations with specific initiatives that will positively impact access to quality health care for rural residents of CT.

The 2009-2010 grant funds are to be used to develop, enhance, implement, or evaluate current or new rural health care initiatives. Grant activities must show a collaborative or coordination of community, regional or state rural healthcare efforts. The application must describe these partnerships or collaborations and how they enhance the success of the proposed initiative.

CT rural hospitals, rural clinics, rural health care providers groups, agencies or organizations with a health care mission are eligible to apply. CT rural colleges/universities and AHECs with health programs serving rural CT communities are also eligible to apply.

Funds cannot be used for direct patient health care services. One to three grants will be awarded for rural health care initiatives. Total grant funds available are $39,200.00.

Applicant must be a public or non-profit organization and must be located in a designated rural area as defined by the 2004 CT-ORH definition or demonstrate that a minimum of 75% of the proposed project activities are focused on serving CT rural residents. View list of designated rural towns.

Applicants may request funding for one initiative only. All applicant awardees must agree to share information and outcomes of their initiative.

Application Process

This is a competitive grant process. All grant applications will be reviewed collectively by a committee, with grant awardees being notified approximately 30 days after the grant deadline. Applications must be typed and submitted on the organization's letterhead.

Questions must be in writing and may be submitted to the CT-ORH via e-mail to Mary Winar, Projects' Coordinator, at mwinar@nwcc.commnet.edu or through fax at 860-738-6443. Questions will be accepted until seven days prior to application deadline. Question responses are available upon request to the CT-ORH.

Anticipated funding period is to be October 1, 2009 through June 1, 2010.

Use of Funds

Grant funding will be issued on a reimbursement basis only. Grant awards may only be used for the objectives for which the grant was awarded. Funds may NOT be used for:

  • Direct patient care
  • Purchasing equipment
  • Lobbying activities
  • Construction or remodeling of facilities

Submission Criteria

All applications must include a CT-ORH grant application face sheet with the total length of application not to exceed ten (10) pages, including the face page and attachments, which may include letters of support. Incomplete applications or those not following the criteria will not be considered for funding. All applications must include the following in this order:

  • Title Page Face Sheet
  • Table of Contents
  • Overview of applicant's mission and role in health care
  • Introduction of project with description of need
  • Clearly stated initiative with goals
  • Objectives with measurable outcomes
  • Description of evaluative process to be used for the initiative
  • Itemized budget submitted on provided budget form
  • Budget justification that aligns with project objectives
  • Project start and end dates with timeline
  • Qualifications of principles/staff for proposed initiatives

The budget justification should include a statement regarding any income that would be generated from the initiative and a statement regarding use of any other funds that will support the initiative.

The submission deadline for grant applications is August 26, 2009.
